Tus alias no están disponibles para MySQL cuando evalúa por primera vez WHERE
cláusula.
Pero su consulta en realidad es igual a esto:
SELECT `users`.`firstname`, `users`.`lastname`, `users`.`id`,
CONCAT(firstname," " ,lastname) AS `firstnamelastname`,
CONCAT(lastname," " ,firstname) AS `lastnamefirstname`
FROM `users`
WHERE (id != '15')
AND (firstname LIKE 'a%' OR lastname LIKE 'a%')
Entonces su concat en la cláusula where es innecesario.
