No es lo ideal, pero puedes calcular la longitud de los elementos de la cadena con esto...
(SELECT LENGTH(permissions) - LENGTH( REPLACE(permissions, ',', '') ) + 1)
Básicamente, cuenta todas las comas en la cadena y las usa como el número total de permisos aprobados.
